Introduction

GUSTO and S-PRESTO studies are in collaboration with National University of Singapore (NUS), the National University Health System (NUHS), KK Women’s and Children’s Hospital (KKH), and Institute for Human Development and Potential (IHDP) of the Agency for Science, Technology and Research (A*STAR).

Form Application System (FAS) is an internal study governance system.

Form A	To request datasets and/or biospecimens	2nd March 2022
Form B	To request to publish and/or present in conferences	1st July 2020
Form C	To request the addition of new study measures/data collection	2nd December 2022
Form G	To seek approval for grant application	20th March 2023

Registration

⚠️ To create your FAS account, it is necessary to engage in prior discussion and reach an agreement with our investigator(s). If you are not an existing GUSTO / S-PRESTO collaborator, please fill up the expression of interest form on https://www.gusto.sg and we will connect you with the appropriate cohort investigator to initiate the conversation. If you are from institutions outside of KK Women’s and Children’s Hospital (KKH), National University of Singapore (NUS) / National University Health System (NUHS), Institute for Human Development and Potential (IHDP), you will need to indicate the investigator you are collaborating with in the registration page.

You will receive an email notification once your account is validated by the Cohort Rep and ready to use.

This takes at least 3 working days.
